A fatal error has occurred. Please contact support.

Good afternoon.
- I'm using:
# Appserv http://www.appservnetwork.com/index.php?newlang=portuguese;
# Windows 7 - 64 bit;
# Firefox Browser;
The installation was successful, the database tables etc ... When I open the 
application it shows me the following error.

A fatal error has occurred. Please contact support.
Fatal error: Call to a member function prepare () on a non-object in C: \ 
AppServ \ www \ simplerisk \ includes \ authenticate.php on line 462

- In code 462 exists following:
         $ stmt = $ db-> prepare ("SELECT date FROM sessions WHERE 'id' 
=: sess_id");

What's the problem? How to fix?

What steps will reproduce the problem?
1. followed the guide
2. worked great until phpmyadmin install
3. Web interface to phpmyadmin does not work (requested URL was not found on 
server)

What is the expected output? What do you see instead?
Should be able to manage the DB through the web interface at this point

What version of the product are you using? On what operating system?
Ubuntu 12.04

User Management

What steps will reproduce the problem?
1. Created a user with an email address as username
2. Logged out
3. Attempted to log in with that account

What is the expected output? What do you see instead?
Login success

What version of the product are you using? On what operating system?
20130916 - on Ubutuntu 12.04

Please provide any additional information below.
No users show up in "View Details for User". I also can't find any way to 
delete a user in the interface.

Embedded ?> PHP script terminators break PHP syntax

My installation is rather unique, so you might have issues supporting it. I'm 
using Gentoo Linux.  I've got PHP, MySQL, Apache, and PHPMyAdmin installed and 
functional.  I've deployed the database components as specified in the LAMP 
simplerisk guide. I can load a basic phpinfo.php script with no issues, so I'm 
fairly confident PHP & Apache are both configured appropriately.

However, your PHP code in the index.php and reset.php files (probably others) 
do not render properly. I suspect this is due to <? ?> PHP code snippets 
contained within other <? >? - within the main code body.  I'm not a PHP expert 
by any means, but this seems to violate basic syntax, from what I've read 
(e.g., http://php.net/manual/en/language.basic-syntax.phpmode.php).

I had a look at setting short_open_tag in /etc/php/apache2-php5.5/php.ini to 
"On" (the default is "Off"), but that was not helpful.  Not quite sure how to 
proceed at this point.

Using clean installation Ubuntu 12.04 LTS
Installed latest LAMP
Downloaded:

simplerisk-20130718.sql

Please provide any additional information below.

I am stuck why it is not working. Used guide step by step.
