It's not clear when a character is consumed, is it discarded from the input stream or not?

The spec doesn't say to discard. However, https://infra.spec.whatwg.org/#algorithm-conformance applies:

Conformance requirements phrased as algorithms or specific steps may be implemented in any manner, so long as the end result is equivalent.

I believe that applies to your other questions as well. The spec needs to define things in some way that is well-defined. If you wish to implement it differently, that is ok if the end result is the same.
