In a modal overlay, a form appears with pre-populated Title, Tags, and Description text boxes. The user can CANCEL or CREATE WEB MAP.

The resulting web map has the same basemap and effects as the app, and all years (each as one layer). The currently active year of the app is checked by default in the resulting web map. Others are un-checked.
