You can simply clone/fork this repo and setup this case on examples in case you're using Bootstrap 4 or you can setup this case on "bootstrap5-examples" branch of this repo in case you're using Bootstrap 5. Once you have this ready simply link it here, reopen this issue and I can check it out. Thanks for your understanding.

Nevertheless my first though would be that something is not OK with your template in relation to Bootstrap. It's not logical that the package is the reason for Checkbox not rendering as it should.
