The SimpleRoleProvider.RemoveUsersFromRoles method contains the line:

int rows = db.Execute("DELETE FROM " + UsersInRoleTableName + " WHERE UserId = " + userId + " and RoleId = " + roleId);

which should be:

int rows = db.Execute("DELETE FROM " + UsersInRoleTableName + " WHERE UserId = @0 and RoleId = @1", userId, roleId);

as the first one throws an 'incorrect syntax' exception when used with Guids.

To call this method, the "Membership.Provider" property must be an instance of "ExtendedMembershipProvider".

In web.config

Both ConfirmAccount override methods in the SimpleGuidMembershipProvider classes try to parse the selected UserId to an int type instead of a Guid.
