Forward http requests to https

All requests to any of the reverse proxies should redirect http requests to https.

Wrong ownership on /workspace/.vscode

When changing themes or another operation that requires writing on /workspace/.vscode/settings.json might fail because the file is owned by root instead of openvscode-server.

Include the SSL reverse proxy in the same image

The hvscs.sh/hvscs.bat script today uses two images:

the main hvscs and
a reverse proxy to provide HTTPs access.

It would be "more compact" to have the HTTPs access being provided from inside the main hvscs image without the need for the second, reverse proxy, image. Nevertheless this reverse proxy in the main hvscs image should be optional to allow for scenarios of running it in Kubernetes where some ingress will actually provide the reverse proxy.
