If the internal storage Map instance is overridden using setIndexStorageObject(), and the get() and set() methods return thenables, then the LiveIndex instance should work asynchronously.

• Asyncify fileAndPositionForIdentifier()
• Asyncify readStreamBetweenIndexes()
• Asyncify insertLink()
• Switch files tracking to be JSON-able
• Switch to using index storage for files map
• Update minimum node version to 0.12
• Update docs with async API

These changes require a fairly large restructuring, and switching to a fully asynchronous exposed API may be the best option.

Something like: .readStreamFromStartIndexToEndIndex(startIdentifier, endIdentifier).

Should return a readable stream starting at the offset specified by the index.
