Add instructions on branching from master before committing any changes

Many are used to commit to master and then PR from forked and amended master.
This results in:

if there is any PR open changes are appended to that open PR even if those logically belong to a different feature (task)
changes can be approved in ALL-or-NONE mode only

Based on this PR case

Quick-fix solution is to instruct the contributor to:

branch from master before committing any changes and never commit to master
add usptream remote to their local clone of forked repo
update local master from upstream and merge into feature branch before PRing
