Clean up source code and comments

  1. Remove all #includes that aren't needed. I know that some of the ISR.c files contain #includes to non-existent header files.
  2. Remove all unused variable definitions/declarations. Again, some of the ISR.c files contain referenced to extern variables that are not used in the .c file.
  3. Clean up all unused commented code. Commented code makes it confusing to read and understand the code. If you really need to keep commented code, you must include a comment explaining why and what it does.
