[reference_wrapper] support reference_wrapper<T> for incomplete T
Add an option to switch off the introspection used in forwarding_base to make reference_wrapper usable with incomplete types.
[environment] extend API for finer control
Need to implement environment::parse_command_line and add class members for the following features.
- Direct test output to somewhere other than std::cout.
- Explicitly include/exclude scopes
- Adjust default log level and per-scope log level
- Adjust per-scope exception rethrowing behavior
- Allow extra arguments to be forwarded to a scope
