I added a note to the README to indicate which options are only available when you drop down to the Ruby interface (width, height and padding)

The command line script is deliberately kept minimal to keep it simple, so only the common options are available. The script itself is pretty small and easily extended.

Hopefully, the need for the width and height options are rare. I think its best to ensure your source image files are the correct sizes manually in photoshop (or other) because having a library automatically center an image can lead to off-by-1px alignment issues if the image size is odd but the specified width/height is even (or vice versa).

I've always found it best to get the images correct first, then sprite them up as-is.
