The bot doesn't seem to be filling in the lead/notetaker fields in the all-hands template. This appears to have been happening since early December (<-- may have also happened the week before that issue).

The bot should automatically fill HackMD.io notepads with a template. Unfortunately, I do not think this is currently possible.

Date in the issue title does not correctly show the next Monday. Example:

Run the bot on Thursday Apr 5 2018.
Expected result: Title = "Monday Calls: Apr 09"
Actual result: Title = "Monday Calls: Apr 09-15"
