This is intended to be an absolutely barebones iOS PhoneGap app that uses the reference implementation of a custom plugin: Echo (as on https://github.com/thegreatmichael/phonegap-echo-plugin-example )
cordova create hello com.example.hello "HelloWorld"
cd hello
cordova platform add ios
cordova prepare
cordova plugin add https://github.com/thegreatmichael/phonegap-echo-plugin-example.git
After doing these things, go to /www/js/index.js and add the exmaple usage from http://docs.phonegap.com/en/edge/guide_hybrid_plugins_index.md.html#Plugin%20Development%20Guide_sample_javascript as in https://github.com/thegreatmichael/phonegap-app-uses-echo/blob/master/hello/www/js/index.js#L49
Why, when I run this, and follow along in Safari's WebInspector, do I get the following output?

There should be an alert!
