I'm able to successfully authenticate to both MediaWiki and Duo, but following the end of the normal Duo web authentication output, there's no redirect. If I manually re-direct myself to a new page, there's still no valid MediaWiki session present.

Having thrown some debugging into SpecialDuoAuth.php, best I can tell, only the conditional at line 20 is being called. The other same-level conditionals on lines 38 and 49 are never called. My guess is that because the conditional at line 38 is never called, there's no session finalized to MW and nothing to redirect me.

This is tested on CentOS 6.0 with PHP 5.3.2. SELinux is disabled, I also tested without APC being enabled, but there wasn't any different result.

Hello, I would love to start using your Duo product with my Mediawiki. I have followed all of the instructions and have ran across two issues which I will list below.

Issue 1) After successfully entering my credentials, I am prompted with the duoauth login screen. No matter when I change in the Duo-Frame.css, the login is still cut off.

Issue 2) After selecting the "Send Me a Push" option, I get the notification and approve the login. The wiki screen changes and goes back to the login screen repeating the process.
