Currently, gitignore is not working due to the incorrect name. The correct name of the file should be .gitignore

Export function from utils folder and import (as well as use) in the target file.

Add appropriate instructions for running the extension locally, in README.MD.

Description of the bug:
Every time the command is run, the code actions made to refactor the repeated code keeps on accumulating. Quick fixes show the quick fix of the entire document and not only the one we asked for.

Expected behaviour:
Each diagnostic for a repeated block of code must show code action related to that block only not for all the blocks of repeated code. Also, as we run the command again it should dump the previous code actions and provide the new ones only.
