The greenway example is all one line and could use some formatting. I also think they have updated their ccda since this was posted. I'd be happy to update it if you're interested.

It's important that the documents submitted be made available under a permissive license. I'd advocate for CC-BY

I'm working with a client using an EHR called SRS (srssoft.com). I was wondering if anyone had any samples involving this system specifically. If not, I'll be sharing as soon as we start generating CCD files for our client.

We need some clarification for such use of id:

• non-unique id was used on purpose here to mark that some identifiers actually identify the business process that the object represents, and may end up being attached to multiple different objects that all relate to the same real-world process (e.g. observations are made from a single laboratory order)
• non-unique id was just copied and pasted to populate form with some observation data
