non-tiled static image generation (center/zoom level)

useful for hillshading, but could be other data

feature: instead of a http server, batch rasterize tiles to mbtiles or folder pyramid of tiles

This may be impractical for:

Large areas
all tile resolutions (1x,2x,3x)
High zoom levels (16+)

output to MBTiles archive

create static build for linux/macOS, as most people don't want to compile mapnik

compile boost 1.74 libraries for each platform
create a Mapnik build with RPATH set to $ORIGIN for linux, or @executable_path for MacOS
Create an archive that includes boost and mapnik shared object files and dylibs
Should we also do the same for tiff, cairo and harfbuzz?
archive should include default Mapnik XML style and default Mapnik fonts (DejaVu)

Features cut off at metatile boundary

segfault when running with no arguments

Mapnik template preprocessor

Implement a preprocessor for Mapnik XML to ease writing filters and zoom-dependent styles.
