3. Upload image - it uploading ok
4. Then ok
5. No image appears, just a blank container
   

• the survey should be able to generate a bar graph showing answers which is shown on the dashboard for users who have submitted previously

It's not quite understand what should be done to save answer.
It is saving now by pressing enter on the keyboard. Possible would be great to save it by pressing some additional button (+) or loosing focus from the field.

Possible would be great to change it to Open or Active

Description field in IE could be a bit larger like in FF

When you have created survey without start/end dates, and finish it, it wouldn't gain "done" status. And "start" button will no be disabled as it should be.

How do you think, is radio button would be better for this purpose?

It could be located on the left or on the center

When you're editing yes/no answers it gives you an empty thing like on screenshot
