I can't run index page because i've this message "ERROR: Unable to open session."
Try launching LTI from moodle and separated domain, but i can't run it, ever same message. (administrator path works fine).

The following code references the wrong table "lti_resource_link". This should be updated to "lti2_resource_link". Since this is tabel which is now created for The LTI-Tool-Provider library (see https://github.com/IMSGlobal/LTI-Tool-Provider-Library-PHP/wiki/Installation)

I recently installed the Sample Rating LTI tool with success. I created a new tool consumer that is active and tried creating an external learning tool link to my application however I can never get it to connect the LTI information to the application. My error message that appears on the Launch URL screen is "Sorry, there was an error connecting you to the application.". Im guessing this means no information is being passed from the LE to the app. In the LE settings I have everything setup to send this information over. Not sure why it wont connect.

We based the tables of our implementation of the LTI-Tool-Provider library on the mysql table autocreation. In it a nonce value is limited to 32, which is too short for some implementations of LTI.
