Yep got details from the settings as described. The only difference is we use forms from D365M (Marketing), not sure there is a difference though? the URL in settings is slightly different, "crm11" as opposed to "crm4".

The issue appears to be caused by an attempt to connect to an instance of Dynamics, with a user that does not have enough permissions. When the access token is retrieved, we use it to get user details, at which point the exception occurs.
I have released a new version of the package here which handles this scenario.
Let me know how your tests go.
