This extension installed in ~/.local/share/gnome-shell/extensions/.
It is now working after remove extension file on extension dir and reinstall from extention website.

Hi, where are you getting this error from?
Can you please include the log from journalctl -o cat /usr/bin/gnome-shell? just run this command and post the relevant part related to this extension.

Message "Type name Gjs_add-to-desktop_tommimon_github_com_shortcutMaker_CustomIcon is already registered" appear many times.

Are you sure you didn't installed the extension twice? That could happen if you install it both as root and with your user. Check the directories /usr/share/gnome-shell/extensions/ and ~/.local/share/gnome-shell/extensions/, this extension should NOT appear in both directories.
Did you tested also on a fresh install of fedora 35 with gnome-shell. 41.4-1.fc35? Is important to find out if it is a fedora 35 related problem or something specific to your system.
