which version of puppet do you use? I had similar issue and it turned out that variable $::osfamily was not declared (it was introduced problably in 2.8) instead the OS name is stored just in $::operatingsystem. You can fix it by changing case $::osfamily to case $::operatingsystem. Here's the corresponding code in puppet: https://github.com/puppetlabs/facter/blob/master/lib/facter/osfamily.rb

It might be easier to update to a newer facter version, which will solve this problem without changing anything in the module itself.
