there is a setting Ignore System/Network servers right bellow the DNS Server list
if you have that off (default) then it will fall back to the system dns
as with all Portmaster rules it goes fro top to bottom, so it will start in your case with the two dot, then the doh and if nothing works fall back to the system configured

thanks for raising the issue that this is not communicated well enough I also updated the wiki to be a bit clearer on this.
It also sparked the idea that we might move the additional setting inline into the other servers... but we will see if we can do that in a clean way

Just one more question. From the updated wiki:
The last server in this list is the System/Network DNS server (it is not diplayed)
This implies that Portmaster always circumvents the user-specified DNS servers when they cannot be contacted (provided the "Ignore System/Network Servers" setting is still off). This in turn implies that having an empty slot in the DNS server config is unnecessary.
Thing is, I set up someone else with Portmaster with the default Quad9 DoT settings and β even though "Ignore System/Network Servers" is off β they were not able to browse the web in a restrictive network environment. If Portmaster really does always circumvent the user-specified DNS servers when they cannot be contacted, why did browsing not work with the default Quad9 config? Curiously, when I removed all the DNS servers, the problem was gone.
