Hi.
Just tried this example and can't reproduce the problem. Please, check in HTML, if your my_actual_attribute field is rendered as select with options inside.

@RomeroMsk thank you for writing back. I was able to find the source of the problem. The error occurs when you have two inputs for the same model attribute on the same page. I believe the issue is somehow related with non-unique element identifiers or names.
Anyway it seems to me that this is not actually a widget issue, so excuse me for bothering you.
