Good afternoon. Sorry for the delay to reply. I've been very busy for the past few months.
I use the "custom: button-card" to create these titles. It was the most efficient and customizable way I found to create my way. You can check this out in my repository named my-home-assistant.

I didn't quite understand your second question. If you're asking what icon I'm using, the answer is "mdi: lightbulb-outline". Found at Material Design
