RTD only updates things in your documentation. Your README is in your top-level project. You would need to copy your readme into your project, (maybe symlinking would work, haven't tried) for it to build.
If you build your docs locally with make html
for example, you will be able to see what it should look like on RTD.

Running "make html" works fine on my local copy. docs/index.rst includes ../README.txt. I can move the file into docs as well but I'm still confused as to why the initial build works but subsequent builds do not.

I did not make any changes to the docs folder or README.txt; however, I did commit another change just now and checked readthedocs.org. The new documentation is up there! Maybe there was a temporary glitch last night. Either case, I consider this resolved.
