Should the "--first" or "--repeat" options influence this behavior? It seems to me that the current behavior (of only showing the first error) should only occur if "--first" is used.

The --repeat
is now deprecated.
The behavior of --first
is to yield only the first error for each code, globally. It means that the error will not be reported any more in this file and other files which are examined in the same run.
The current behavior of E241/E242 is to report only the first error on a logical line (which is enough to fix it manually), however it can report the error later in the same file (except if --first
is added).
