I'm not able to reproduce, but that is likely due to the other bugs you mentioned which are now fixed in master. I'm using the following to test (in a fresh AWS account):
$ KEY_PAIR="test-poa" TF_VAR_db_password="abc123def456" TF_VAR_new_relic_app_name="poa-explorer" TF_VAR_new_relic_license_key="1234-5678-09AB" bin/infra provision
I'm exporting all of the config vars which you would be prompted for interactively, this way I can just run it and come back at some point. On current master, I just ran the installer on a clean AWS account and it successfully completed (well, I got a transient Terraform error which is apparently a bug in Terraform, but re-running provision fixed it, unrelated to this issue though), so I think if you pull the latest changes and run it again, you should be good to go.
I'll close this for now, but if you encounter this issue again, reopen it and we can dig in further!
