Please make sure to install the application on a clean database with clean cache folders.
If the installation failed for any reason, make sure to wipe clean the database and cache folder before re-trying the installation again:

 rm -rf app/cache/{prod,dev,sessions}
 mysqladmin drop your_db_name 
 mysqladmin create your_db_name

And check app/config/parameters.yml to make sure that the installed parameter is set to false as well.
