Is this a DISCON.IN that you have tuned yourself, or one that is provided? Which turbine model are you running? Can you provide more details of the simulation?
If you export your OpenFAST .out files you should be able to plot any channels - you may be able to notice rotor over-speeds, massive tip deflections, or any other weird controls-related behavior just before the fatal error.

Following some indications and tests from Dr. Jonkman, I discarded the controller being the culprit of this instability, although I am still unsure about why would the issue happen only when VS_ControlMode > 0
