Here is same issue with your demo site. Single thing that I changed is addition of '_' (underscore) to form class.

What Operating System and version of your firefox? Have you checked if the same happens in the bootstrap page in your browser?

imo it does not depend on OS/Browser. Its because of additional css that is applied to input if form has that (or another form-related) class. I wrote about that in OP

It is recommended to use this structure, precisely because the form is written that way in the bootstrap. Change an alert inspect the code is pretty obvious that it will present problems.
