There's not really much I can do about this one. miniaudio just takes what WASAPI gives it and then passes that on through to your data callback. It'll do some data conversion and resampling to match what you request in the device config, but nothing else. If you leave the format, channels and sample rate unset in the device config (leave them set to their default values), miniaudio will simply pass it through with no conversion whatsoever. You're just stuck with whatever WASAPI gives you.
I've read somewhere (I can't remember specifically where it was) that WASAPI can be inconsistent with how it gives you the audio data. Sometimes it'll give you data that's at full volume, and other times it'll be scaled based on your current system volume. Overall I think loopback mode tends to be a bit inconsistent. I wouldn't be surprised if this is another one of those annoying inconsistencies.
Just to eliminate any potential errors on your side, have you run the simple_loopback example?
