Prometheus metrics should have an "instance" dimension in all cases I've seen so far. I've been using that to distinguish between hosts, as the instance will usually contain the host name.
Unfortunately the "instance" is not able to be set straight into "host" in the abstract case, as an instance may also contain more than just the hostname -- e.g. hostname/pid for example.
