Hi Brian,
Headless launches Xvfb asynchronously in the background, so it could definitely happen that the Ruby program tries to use the framebuffer before it is created by Xvfb.
I added an existence check to the launch process; please try if fix in the wait-for-xvfb-to-launch branch resolves your issue. To do this, change the headless reference in your Gemfile to
gem 'headless', :git => 'git://github.com/leonid-shevtsov/headless.git', :branch => 'wait-for-xvfb-to-launch'
from headless.

After a few weeks of using this, I have not run across this issue again on my CI server. I would recommend merging it into the master branch.
