@ericwan78 an example would be when you open an application that takes a few seconds to load. While the application is loading (say the screen is black or a loading "ring" is showing up), the accessibility service highlight remains visible. That is, the bounds of the node representing the icon you just "clicked" on, are shown on the screen. This "feels" like something is broken when using the application.
