The plugin doesn't restart after the reboot. This happens on ubuntu 14.04 with the latest plugin version. I have tried an older version of the plugin on ubuntu 12.04 and it worked fine: it auto restared whenever the server rebooted.

The solution to this problem is that instead of : dokku postgresql:create , it should be : dokku postgresql:create <app_name> . In the later case it works fine after hard rebooting.
If you name your database anything other than your app name, then it wont reconnect after reboot.
It should be changed in the readme.
