What game are you playing?
Issue 1
The first issue should be easy enough to fix, although I'm unsure as to why it is occurring. Were you playing on a server where there were multiple players with the same username?
As a possible temporary fix (i.e. probably won't work, but worth a try), you could try adding your steamID3 to the properties.txt file. Visit the link for instructions.
This issue will for sure be fixed in the next update.
Issue 2
Could you either send me a copy of a console.log file from when you were playing in this server or invite me to play on the server.
When you say "unable to control the music," do you mean unable to request songs or unable to perform admin tasks with SourceRadio? If it's the latter, please note that being an admin of a server isn't the same as being an admin of your instance of SourceRadio (see admins.txt).

Yes, it's the latter; I have to use their name instead of their SteamID because the server automatically puts on the [Admin] tag on the user. It's worked for other admins, but some of them are unable to skip songs. I'll send a copy of the console.log when I have time.
I'll also try your temporary fix for now as well; if it doesn't work I'll respond back.

I can't be positive without a log, but I'm guessing that when a server admins send a message, it shows up in the log like this "[Admin] Cool Guy42 : !song darude sandstorm". This is a problem because as a normal user, I could set my username to [Admin] Cool Guy42, so it's not safe to just cut off "[Admin]" from the beginning of usernames.
What do you mean when you say "I have to use their name instead of their SteamID?" Also, could you send me an IP or invite me to one of these servers?
Does Issue 1 happen when you're on a normal server (e.g. Valve server)?

Hello.
For some reason, I can't use any admin commands. I'd figure I'd add it to this thread.
I get no errors, it's just not working at all.
Also, the fix did work until it broke... again.
EDIT: Fixed the issue; the SourceRadio things have to be at the bottom of autoexec.cfg, nowhere else will work.

Thanks for reporting. I'll add a comment to that line explaining it should be kept at the bottom.
