No need to define the fallback yourself, otter already falls back to the regular lsp action if not within an otter context. Try calling the otter function directly to be sure you are actually using it. e.g. :lua require'otter'.ask_hover().

In the past I have also noticed problems with plugins that overwrite vim.ui.input, which is the default for getting the new name for the rename and I'm also using it here

vim.ui.input(prompt_opts, function(input)

This might be a problem in your case, since I noticed this custom text input box. Try disabling this plugin to check.

Lastly, check if otter is actually activated by e.g. checking for the hidden buffers created by otter with :ls!.

Other than that I need more context. What filetype are you using otter with? What type of code is embedded in it? What language server is configured for said embedded code?
