You don't actually need to wait for the ready event to append elements to the DOM. You just need to run your script after the elements that you're selecting have been loaded into the DOM. So the common example of appending to "body" works great like this:

<body>
<script>
d3.select("body").append("…");
</script>

Note how <body> comes before the script? This applies to any other element, too. So the following will not work:

<script>
d3.select("#foo").append("…"); // this won't work
</script>
<div id="foo"></div>

I prefer to put scripts at the end of the document anyway so that they don't block the initial rendering of the page. This has the nice side-effect of allowing elements to be selected immediately in the script without having to wait for them to load.
