Thanks for the report! Depending on the operating system and browser you're using, notifications may not appear more than once for a given organization until they are dismissed, e.g. if you visit columbia.edu and then visit cuimc.columbia.edu, you may only get one notification. This seems to be something some browsers do to prevent spam. Perhaps that's what's going on here? One way to check would be to close all your browser tabs and windows, then launch a single window with a single tab in it and go straight to https://www.cuimc.columbia.edu/ and see what happens. You should get exactly one notification when you do this. (Thanks in advance for giving this a try!)

In any case, for your reference, the little exclamation point just indicates that you should have received a notification; it's a fallback indicator just in case a notification doesn't appear for any reason.

I just tried restarting the browser, spooling up a tab at https://www.cuimc.columbia.edu/, but alas no notification.

Thanks for giving that a go and apologies for the continued trouble! Have you received notifications from other browser extensions before? I'm wondering if maybe something is preventing them from appearing, e.g. another browser extension or some setting you've inadvertently got configured in your browser's settings is blocking themโ€”or perhaps something at the operating system level is blocking them, like anti-virus or anti-malware software. But if you've gotten notifications before from other browser extensions, all that would be unlikely.

Another thing you could try just as a debugging step would be to try a different browser and installing our extension on there, where we currently support Chrome, Edge, Firefox, and Opera. That would help rule some things out.

I'm still not able to reproduce on my end, but I'll keep trying. In the meantime, you could pin our browser extension's icon to your browser's toolbar (if you haven't already) so that it's always visible at a glance and look out for that exclamation point, but that's definitely not as ideal as getting a notification as well.

I finally found out what's up! I had focus mode enabled on windows, and the extension sends out a windows notification (see image).

So it was windows that suppressed the notifications.

Thank you very much for following up with this! Glad things are working for you now. I'll add something to the README shortly that calls this setting out in Windows so that others can benefit from what you've found. :)
