There is no restriction on how many instances of Illuminatech\Balance\BalanceDb can be created.

You can add extra instance using another key in DI container and use it at will.
For example:

// define extra service via service provider:

use Illuminatech\ArrayFactory\Factory;

class MyBalanceServiceProvider extends ServiceProvider
{
    /**
     * {@inheritdoc}
     */
    public function register()
    {
            $this->app->singleton('my-balance', function () {
                $balance = (new Factory($this->app))->make(array_merge(
                [
                    '__class' => BalanceDb::class,
                ],
                $this->app->get('config')->get('my-balance', [])
            ));

            return $balance;
        });
    }
}

// extra service usage:
app('my-balance')->increase($accountId, 500);
