Well, I got it fixed, but had to rip out the DbProviderFactory, change all DbCommands to SqlCommands, add a reference in my project to Microsoft.SqlServer.Types (from Sql Server 2008 install), in AddParam() check for SqlGeography item type and finally add UdtTypeName == "Geography".
Quite a lot of "bastardization" of the original Massive code, not sure if you want to check it out or not. Let me know.

It would be great if a MassiveSqlServer version could be maintained in this repository instead of starting a new project.
This could still be lightweight and
- use SCOPE_IDENTITY() rather than @@IDENTITY
- support spatial types
- rip out DbProviderFactory and use SQLCommands.
I removed DbProviderFactory as well and am using SqlCommands.
Profiling showed about 14% performance increase for my queries!

hmic a SQLCommand is a DbCommand. Not sure what profiling you're doing but...
