You probably have dvrpatches which is known to crash with the Witcher 3. That is not supported.

Remove dvrpatches and it should work again.

Indeed, after uninstalling SysDVR the game works again but SysDVR will no longer work with this game permanently? 😥

Yeah streaming the witcher 3 is not possible at the moment.
