Turns out it was related to my region settings.
I'm using "Norwegian bokmål" as region format and the access token/refresh token expiration values were returned from KeyCloak like 09/15/2015 23:55:27, which caused DateTime.Parse to fail in ValidateCookieIdentity.
I'll send a pull request which uses CultureInfo.InvariantCulture when parsing the access token/refresh token expiration values.
