no longer a bug since this is disabled; renaming to reference issue of "below all the same" annotations

we need a much more sophisticated strategy for this — using the META
span type is the first step. probably we will build a "cache" of annotations that have a TTL specified by the meta span (e.g. "until the end of the commentary" or "until the end of this hexagram") and then, when we come across characters for which there's no annotation, we check the cache first.
