Fixed Conference Refcount for Participant

Only partially.
Still working on the +3 participant / foreign channel (sip) participant
issue. Only fixed a refcount.

SIP / Participants +3 Fixed
Fixed by removing AST_BRIDGE_FLAG_SMART
The smart flag would try to create a new bridge channel which only holds the remaining participants and destroys the previous one. This resultet in a segfault, during old_technology->destroy (inside smart_bridge_operation).
Without the flag everything works fine. We might loose a bit of performance on some level, but i am not even sure of that.
