and bug occurs earlier. But interesting to note that the x and z locations changed but bug occurs at nearly the same y location.

I noticed a slight difference between my input deck and the default one in examples/eigenvalue/2d_c5g7/
. I was placing the source over the full problem and the default input deck only places a source in the assembly regions. Ran it again and am still getting the same bug, but the particle is stuck in x position, not the y. Also note that I'm running with the old 2d c5g7 cross sections, not the newer TD cross sections which contain delayed neutrons. This shouldn't affect the bug, but may affect exactly when it occurs.

@clemekay suggested moving the x=0 and y=0 boundaries to values slightly greater or less than 0. I set x=1e-10 and y=-1e-10 and the simulation completed without error!
