A possible reason is bad firmware 11.608.12.10.209 in Huawei E1550. This 
version of firmware allow voice calls in Windows with Mobile Partner, but seems 
not to work in Linux :)

Another reason is the lack of power for device at peak load when call is 
starting.

It is not a firmware problem. Just updated to 11.608.14.15.311 and still same 
problem

Outgoing call, everything is fine.
Incoming call, no voice from GSM party.

Strange, but on incoming call format is slin, and it not changing to gsm. May 
be this is a problem.

About the CLI I was having same trouble on incoming calls with the absense of 
CLI and solved with adding this line:

[rede_movel]
exten => s,1,Set(CALLERID(all)=${CALLERID(num)})

The CLI was coming as num and the sip phone was only showing name for the CLI.
