Moreover I can say from the logs that insertions of the same data happens from 2 different threads simultaneously:

From this we can see that the first cache put succeeded and the second failed.

I spent quite a bit of time trying to reproduce this but wasn't successful. If you could provide a reproducible sample, that would help a lot.
