Videos served only on the Flash player do not expose parameters when not logged in, e.g. https://www.nicovideo.jp/watch/nm11960162:
HTML 5 video watch page cannot be used on your current system.
Please switch to Flash version after logging in.
This also means we can't request the Flash player when needed for high quality. This being the case, I think the best way to handle this is to display a warning when not logged in and download videos when we can. When #24 is fixed, this will prevent a lot of low quality downloads being grabbed when not logged in.

How --no-login handles each video type:
- Flash video only available on Flash player: not available for download, raises ParameterExtractionException
- Flash video on HTML5 player: downloads low quality .mp4 re-encode
- MP4 video on HTML5 player: downloads highest quality encode available
